Dont really know if this is the right place to post this. I have a old Stevens 94 that I dont really use. The gun is in pretty rough shape so, I was thinking of turning it into a Leopard gun. What ATF form would I need to fill out to make it legal?

Call a NFA firearms dealer and ask. I think it is a form 1, (don't know why that sticks in my mind, could be totally wrong.) You are requesting to manufacture a short barreled shotgun or rifle. After you pay your $200 tax and get approved, AND ONLY THEN, you can cut the barrel and/or stock to make it any length you want. You will have to get the receiver engraved with who manufactured it, date, and city/state, I believe.
Once you do this, that serial numbered receiver will always be an NFA weapon, subject to all the paperwork and the $200 tax if you ever decide to sell it, even if you put a standard length barrel back on it.

Not entirely true. Once it has a stock you can't make it an AOW. However, you can make a short barreled shotgun without a stock. It is just a $200 tax stamp rather then the $5 AOW stamp. I don't know if I can post a link to another board here or not but PM me and I will give you the CIII board to ask the question.

an existing shotgun into a Howdah would be a SBS, you would fill out a Form 1 to create an SBS, pay the $200 fee, only when you receive the approval can you cut it down.

thank you everyone for your replies. I called the National Firearms Act Branch and they directed me to Form 1, downloadable in PDF form. I got the paperwork and the check and will send it out today.
