This sounds like a cluster all around. So the OWS officer tried to eject but failed to eject, and NO ONE else attempted to eject as in no order to eject was given.

Hummmm...

Anywho, shame we don't have another 100 B-1B's in inventory... and some highly trained crews to man them.

Couldn't have been too much of an emergency, they got it on the ground successfully, saving a mere $280 plus million dollar aircraft. Looks like a good job by pilot.
