Just get an O/U with removeable chokes, and put a rifled chocke in the bottom barrel.

I have 2 of them, 1x Beretta 686, 1x Fabarm.
Both called "Paradox" guns.

The exact details escape me at the moment but IIRC; Frankonia had an Italian made O/U shotgun in their catalog several years ago that was advertised as a Slug Double Rifle.

There was an SKB over under sold on Gunbroker last year like this.
But I think it was a custom.
