This may be somewhat of a stupid question, but why do soooooo few boot makers make boots in 15 extra wide? It has reached the point where I will have to go to a custom boot maker to get a pair of boots on parr with the ones you are listing.

Well, it's probably cheaper to do it on a limited basis then to offer production shoes to a very limited market. I've never seen a 15E shoe myself...must be awesome.
