Wasn't that the one where his PH was shown to have screwed up before too?

Actually, I can almost guess that one can screw up and shoot the wrong animal.

But an elephant!!

That takes some serious screw ups.

I consider George Hoffman to have been a man above reproach(may he rest in peace).

George related in his book that he accidentally had a client shoot a dwarf elephant. If George can make that mistake I would guess that anyone can.

BTW, George paid the TF out of pocket and allowed the client to take another IIRC. George was a true gentleman.
