I can see inletting and shaping with a duplicator, but, what about the guard screw holes? How would those be drilled?

On a 5 axis machine you can line the drill bit up with the hole. On a basic 4 axis the guide and cutter move in an arc so getting a prefectly straight hole is difficult. Cut the mouth and use a drill or press to cut the rest.
