I recently acquired my 2nd Mod 70 safari classic in 416 Rem.
When I load the magazine and work the action, all the cartridges pop out at the projectile end of the box.
Really great for a DG gun!!!
Is this a spring issue, a follower issue or a feed rail issue?

Almost certainly, but without seeing it and fiddlin' with it who could know exactly what it is? If you already have one, try the spring, follower and mag box from the one that works and see what happens.

I would guess follower. Rounds would occasionally pop up like that on my 375. Roger Ferrell added a small spacer to the left side of the follower to correct the problem. BTW, he changed out the mag spring with a non-magnum unit- this allowed an additional round to fit.
