when we are out of the USA ? We can buy Travel Guard, Med Jet, Global Rescue etc, but what about hospital and emergency care insurance?

some cover you overseas
most don't
some offer extra travel policies
Medicare doesnt cover outside the US
Generally, you are expected to pay upfront overseas and deal with your insurance later to get reimbursed.

Insure My Trip I usually buy just the fringe coverages, including Medical. [Excluding the Safari and airfare, much of which may be refundable]
It is pretty reasonable that way.

I had a client get hammered by a buffalo last year in Zim. Travel Guard handled everything including hospitalization in JNB.
