I already have a Form 4457 notarized, with the same equipment I'm taking to Namibia this April. However, I've changed street addresses, but still reside in the same town (if that matters). Is it a requirement to get another 4457 approved with the new address, or can I just use the old one? | ||

Have you notified the State Department and changed your passport info? Likely not so why would you change your 4457? As jdollar said, use the old form. | |||

JGRaider: That's true just a page to pencil in your contact info. I realize my response seemed a bit short and pointed. Sorry. I have moved to another city and have gone to Africa 3 times with the old 4457 and a new one for an additional rifle. No problems or questions. Good luck on your hunt. | |||
