The most disturbing part of that video is the idiot who thinks it's so funny.

When an odd malfunction happens you chalk it up to some sort of mishap or misfortune. You investigate and try to take actions to reduce such failures in the future. When a similar malfunction happens on the next pass you have to assume there is something very wrong with the loading and/or rigging. The DZSO should have aborted the drop after the second malfunction. Instead they continued with costly consequences. Thank goodness things broke loose upon inflation and not extraction or the results might have been catastrophic.

Well, how they had to react, to cry aloud? It's like you're not served in the army. Shit happens, and I'll disclosure a military secret - in any army.
