Just open the Research or mckenzie forms.

Research and Mckenzie just carry closed mouth forms.

Art Line Taxidermy in Denmark carries an open mouth form, but it's going to run around $400 to my door.

sorry, I misread your post. I can do that(alter) but it will take more than opening the mouth. The head angle is wrong for open mouth as well. It can be done though, but I was hoping to find an open mouth form.
