I assume that this form will accompany the actual skull for the monitoring authorities to exam for age certification???

Frostbit, by law, a lion aged at 5 is illegal and therefore subject to the penalties outlined in the Regulations governing hunting.
My understanding is that they will allocate the lion's into 2 categories; suitable or not suitable. The assessing panel will determine which category. If deemed suitable, they will be cleared for export. If unsuitable, the penalties will be applied.

Along with being charged with a Lacy Act Violation (Felony), if you bring it back to the States. Which I am sure the zealots on this forum would cheer your prosecution and life changing punishment for something the other side cannot even prove.

The PH is subject to penalty of a fine and probation for first offense I believe.

I doubt I'll ever hunt Tanzania anyway but I just wondered what the actual penalty was.
