The Dutch have revised the Consent Form since Steve at Gracy Travel was so kind in posting a completed example on his website. It was Model WM 18 A, now WM 18 B (top of form), so, I have questions on 18 B: Sec 4 K, Address of delivery: What to use here? Sec 5 B, Enclosure: Yes? because it's in a case? Almost, I used the earlier version, but looking at my forms and realizing that they have amended the forms to find out where you are "taking" them, I think that it is asking for the address to which the arms will be "shipped." For example, if you are going to be hunting with Game Trackers in Tanzania, I'd put the address of their offices, the contact address they provided you, in that block going out, and your home address (for a third time) returning.
