I don't believe it is on the internet, but I emailed it to you. Note is is a two-sided form, so you will need to print one side - then print the other side on the back of that same sheet. You need three completed originals when you arrive (I also take three blank forms, just in case).

Regards,

Terry

It's available at http://bigfivehq.com/safaris.html, follow the link to "Tips and Travel Advice". All the forms for most of the countries can be downloaded from there.

For those who have inquired. In the past, all I have used is a one-sided form. This season, Swainson's sent me this form which is two-sided. The front page appears to be the same as the single-page form I have used in the past. The back-page is for extending the Firearms Permit, but it has the following language (note the word "overleaf"; which leads me to believe that it is now a two-sided form):

NOTE:- The certificate (overleaf) is to be completed in triplicate. Original to be handed to the visitor, first copy to the Controller of Firearms and the second copy to be held at the Customs Post.

Regards,

Terry
