Just use strong, high quality plates and studs. There are lots of Interweb horror stories about spacers, but you never seem to find actual stories of damage with picture documentation.

About the only thing I could recommend is simply using a torque wrench and ensure studs and bolts (depending upon application) are tight and remain tight. It is always a good idea to recheck torque after the first few hundred miles.

If you're going to carry any significant amount of weight or do any real off-road driving, I don't recommend spacers.
You'd be better off to buy new wheels with the offset you're looking for.
