I faxed my application in early February, waited a month and then found out that for whatever reason I was not "in" their computer system. With the very able assistance of Jamie in Adam Clements office i refaxed and had my permit several days later.

I sent my app in months in advance but didn't receive approval. So, I called Holland and was told (very nicely), "Don't worry. If you haven't received 10 days before your departure, call back and we'll fax it right away." Sure enough, about 2 weeks before my flight I received the permit.
