Have you checked the fit of your barrel's locking bolt to make sure it is engaging the frame's water table deep enough and the same each time? Take a magic marker and color your locking bolt. Then cycle the barrel to see if it is locking the same depth everytime. This used to be a problem with the original Contender frames. The split bolt helped,but anything made to fit anything can usually be expected to fit nothing correctly.

It sounds mechanical, not the gun, but you. I suspect that you are anticipating the recoil and pressing forward as you shoot. To test it, have someone else load the gun and every so often, give you an empty chamber. You will probable catch yourself pushing the muzzle down. When you have the forend on a rest, it limits the effect. This is natural habit and I've had to break myself of this one too. When I'm accuracy bench testing a Contender, I have the pistol supported on the forend and the butt of the grip. My goal is to have the cross hairs on the target with as little hand pressure as possible. I then squeeze the trigger and if all works as planned, it is a bit of a surprise when it actually goes off. Doing this I've had some loads go MOA @ 100 yards. For me this is outstanding.
