Once again, Engine INSPECTION INTERVALS are a major cog in the safety wheel. In this case the High Pressure Turbine Failed which ruptured the right wing fuel tank.

Note: Even frequent Inspection Intervals are not fool proof but greatly enhance the possibility of discovering a defect. Sometimes a failure reveals that a DIFFERENT TYPE of NDT Inspection is needed such as X-RAY instead of Ultrasound.

The RR Trent 1000s are having some serious blade corrosion and cracking issues now. New blades are a year away. Fun times in the air.

What I do not understand is why are they losing containment? I thought these things were designed to contain the bits when one grenades. Seems with every blade failure instance shrapnel is causing secondary damage. And that tends to lead to fuel leaking, hydraulics bleeding and lots and lots of screaming.
