The medical profession of psychiatry must be the most difficult. So, I don't know how they make diagnosis at all. They have standards that they themselves set, and I figure that's partially considering treatment.

However, it seems to me that it would be hard to argue that a person with such traits, especially extreme and inclusive, is well and/or not sick. Sadism is sick, for example.
