Form is filled only if you want to get your Visa prior arriving (via consulate), if you purchase it at the entry (airport) you don't have to do anything - just pass the passport (make sure it is still valid for at least 6 months) to the official and pay the sum.

If you are not staying for longer than six months, you enter Zimbabwe with a minimum of formalities. All you need is a valid passport, a return ticket to your country (or enough money to buy one), and sufficient funds to cover your stay in Zimbabwe.

I went to Zim in 2006. I happened to be working in Washington DC a few months before the hunt. I walked over to the Zimbabwe consulate and got the application and a week later had my visa. Then when I arrived in Vic Falls, I went through the line that had two people in it instead of 100. That was worth the extra $50.

I'm doing it the same way this August since I arrive in Harare this time and I want to leave the airport as soon as possible.
