Amsterdam firearms form

Pardon my ignorance, but how should you fill out this form? I know that I am the shipper but am I also the Consignee or is this my outfitter? Under "Way of Transportation", is the destination Tanz or Amsterdam?

You are the Consignee and the destination is the city in Tanzania you fly to (Dar or Arusha etc.)

Make a couple of extra copiesot the completed form after it comes back from Amsterdam....They will take one off you in Dar on your way home..

Thanks guys,
AR comes thru again.
We booked the airline tickets with Kathi yesterday so we are going. Don't know what we are going to hunt or how long we are going to hunt but we are going.
