z1rIt it drilled & tapped? Try to ensure the holes are in the proper location and sre straight. Rear hole in the front ring should be .500" forward of the rear edge of the receiver ring. Front hole should be .860" forward of the rear center to center of course.
If possible insert screws into the holes to see if they are straight up and down and not slanted forward and back or side to side.
If rear bridge had been altered/recontoured ensure that it is even and not washboard in appearance or dished. All too often this is done by hand not machine and the results show.
