If it's mechanical or female it will eventually do something that cannot be explained.

Never point a gun or a female at anything you don't want destroyed...

I was 18 and hunting with an old two pipe with hammers. The gun had seen a lot of work and was carried by the ‘pig boy’ who would shoot the pigs out of the maze at night. It’s was an African classic in that the barrels which were starting to splay apart were bound together with old bicycle tube rubber.
Anyway I digress...
I had shot a rabbit and as I walked up to it it came to life and proceeded to kick in circles while on it’s side. With perfect timing I butt stroked the rabbit as it passed my feet picked it up and put it in the truck.
When I unloaded a short time after I noticed the second shot shell had a dent in the primer. When I tested the weapon it was apparent the non cocked hammer would in the process of butt stroking fly forward and hit the primer of any shell in the chamber. I was lucky that day!

I was hunting bushbuck carrying the rifle at port arms. I took my strong hand off the rifle to wipe it dry from sweat and just as I went to re-grip it I stepped in a hole. I grabbed the gun funny and my thumb hit the safety back as my ring finger slid inside the trigger guard and tripped the trigger.
I always have the gun pointed in a safe direction but it made Vic and trackers jump.

The closest thing I’ve had to an AD happened during an NRA Highpower Rifle Match. We were shooting Slow Fire prone at 600 yards, and I was leading the Match. My first 19 shots were all 10’s and X’s and I had just needed to shoot a 10 to shoot my first 200 at 600 yards. As I eased into a tight prone position I gradually approached the bullseye from the 6 O Clock direction. Before I was settled into my final sight picture, the shot broke and I shot an 8 out the bottom for a final score of 198. I still won the match but missed a chance for a perfect score.
